After Life Care Hub uses different labels to show what has—and has not—been confirmed about a provider. A listing status is information, not a guarantee or endorsement.
How to read provider listings
Basic public record
The business was added from an identified directory source. The business has not necessarily confirmed the record, and private contact fields stay hidden until the listing is claimed and activated.
Ownership reviewed
An administrator approved an ownership request and linked the profile to an authorized business representative. Claim approval does not by itself mean credentials were verified or that the provider can receive family requests.
Documented provider review
An administrator recorded a completed review of identity or ownership, service category, location, public contact details, and licensing or professional credentials when those requirements apply.
Program participation
The provider has an active participation record. Membership may affect directory placement and introduction priority, but it does not replace verification, administrator review, or a family’s choice.
For verified profiles
What the documented review covers
- Business identity or authorized ownership relationship
- Relevant service category and stated service area
- Public contact information and business website when available
- Licensing or professional credentials when applicable to the service
- Agreement to the provider standards required for participation
What families should still confirm
Licenses, insurance, availability, pricing, scope of service, disciplinary history, and current standing can change. Confirm the details that matter to you directly with the provider and the appropriate licensing or regulatory authority before hiring or sharing sensitive information.
